OCF-1 – Application for Accident Benefits - Complete and return this form to your insurer within 30 days to apply for accident benefits.
OCF-2 – Employer’s Confirmation - Submit this form if you’re off work for more than seven days and are applying for income replacement benefits.
OCF-10 – Benefit Election Form - Use this form to choose between income replacement, non-earner, or caregiver benefits.
